We've had a few reports of this and are looking into it now.
Edit: Looks like one of our upstream carriers for some destinations was not completing calls properly, but since they were not returning an error code (they appeared to go through), the system was not detecting that they were not completing.
We've re-routed calls away from there for the time being and all seems clear now.
As to why no error code was being returned to us, we're not sure but have contacted the carrier in question. When a error code is returned, the system would automatically re-route.
I'm told all destinations mentioned in tickets about this have been tested and verified connecting now.
