Porting paperwork?

I signed up for voipo service a few weeks ago with a new number to 'try things out'. Well, things have been going good and now I'd like to start the process of porting my current POTS # to voipo, but I can't figure out where to start?

Can someone point me to the paperwork I need to fill out to get this going? I looked over my vPanel, and searched the forums, but couldn't find anything.

Send that with a bill copy or screenshot from your current provider to support@voipo.com or fax to 949-829-4201.

Another good place to look for things is http://voipo.com/help/ the forum can sometimes have a delay.
