I have general averages, but looking for specifics specific numbers and a city name.

I use the following services for home, work, and home office, respectively. (For your reference, Tim, you can say the city is Toronto.)
Bell Canada (landline) - every month, there's $0.19 charged for "911 emergency service access" and $2.80 charged for "touch tone service"
Rogers Home Phone - a recurring, non-governmental $5.95 system access fee (also called a system administration fee), $0.22 911 Emergency Service Access Fee (I believe this varies by province), and $0.19 TTY Hearing Impaired Assistance fee. AFAIK the first two fees are waived for the second line on an account. The "All World Plan" (long distance) is $1/mo + usage. (Calls between Rogers customers are free and incur no long distance charges.) (BTW these charges are comparable to my monthly Telus Mobility phone bill which shows $0.50 "Enhanced 911 access charge" and a $6.95 network/system access fee.)
Telehop Broadtalk.ca (rebranding of Vianet.ca's vTone service) - the monthly charge for the VOIP service is all inclusive, except for provincial and federal sales taxes (8%, 5%, respectively).
