PM sent, just tried again, same result.
For what its worth, I also have a distinctive ring set for that same number (the 301-695 one), but I've had this set for weeks.
Brian -- thats actually the exact thing I've noticed -- try disabling it and then try again. Looks like its being sent upstream to PSTN carrier -- which I'm going to fix, but see if disabling it does not fix it right now.
PSTN did not like it in the headers -- essentially.
There we go - that was it! Strange, since it's been working for a while (or at least I think it has...)
Glad it is working now -- I'll go ahead and file a bug report internally, and we'll get this taken care of -- these are part of the reasons Simul Ring is still in beta haha. But glad we know the issue now -- thank you for all of your patience and cooperation!
